How do I make an appointment?

Please see here for our current practice location and contact details. You will need a doctor’s referral before making an appointment.

Your general practitioner can provide a referral that is usually valid for 12 months and sub-specialist referrals are only valid for 3 months. The referral may be faxed or e-mailed to us directly. If the referral is e-mailed, please be aware that confidential information may be compromised in the unlikely event of a cyber attack on our side or yours.

It is helpful if you submit your registration form well in advance of your appointment. You will receive an SMS in the days leading up to the consultation but if a response is not receive by 3PM the day before, the appointment is cancelled automatically by the practice, and re-booking will be required.

Limited parking is available both on-site and in surrounding streets.

What are your fees?

Fees are charged depending on the issues addressed during our consultation and are set by the practice. These are payable on the day of the consultation and a portion will be reimbursed via the Medicare rebate, provided you have a Medicare card and a valid referral.

Late cancellations within 24 hours of your appointment, or failure to attend your appointment may attract a fee.

What should I bring?

Your Medicare card, referral letter, and registration form (if not already submitted).

Any previous professional assessment reports, specialist letters, and investigation results.

Your child’s recent school reports if the concern is about their learning or behaviour. A letter from the teacher may also be helpful.

Their infant health record, commonly known as the “green book” in Victoria, particularly if they are under 6 years old.
